This Athlete Sponsorship Agreement ("Agreement") is entered into as of the date last signed below, between Southern Ethos Limited T/A NRG Fitness, business located at Shop 26, Pines Plaza, Junction, St. Elizabeth ("Gym" or "Sponsor"), and
an individual ("Athlete").
