Membership Freeze 🥶

Parental Waiver & Release

The Parental Waiver and Release Form is required for any participant under the age of 18 and serves as formal consent from a parent or legal guardian. By completing this form, the parent acknowledges and accepts responsibility for their child’s use of the facilities, confirms their understanding of the associated risks, and grants permission for the minor to participate in all approved activities.
